Certificate of Disability for Discharge.
To be used in duplicate in all cases of discharge on account of Disability.
E.F. Small, Corporal in Capt. Jones Company of Kennedy Light Artillery (Independent) of Confederate States Volunteers, was enlisted by Captain Charles P. Jones of the Kennedy Light Artillery at Washington N.C. on the 23 day of Sept 1861; to serve during twelve months. He was born in Beaufort Co. N.C is 17 years of age, is 5 feet, 6 inches high, light complexion, grey eyes, black hair, and by occupation when enlisted a student. During the last months said Soldier has been unfit for duty from a rupture.

I certify that I have carefully examined the said E. F. Small, Corporal in Captain Charles P. Jones Kennedy Light Artillery and find him incapable of performing the duties of soldier, because of Hernia- caused by straining in light or flying artillery Drill.
